Western Tidewater Free Clinic Eligibility Appointment

In partnership with Bon Secours, the Western Tidewater Free Clinic will be hosting eligibility appointments at Hayden Village Center on the first and third Wednesday of each month, beginning September 7th and running through December 21st.
For the Clinic to be able to determine eligibility, prospective patients must have proof of the following:
- identification
- social security or tax identification number
- residency
- income
