Veteran Directed Care

The Veteran Directed Care Program is a collaboration between Senior Services of Southeastern Virginia, Bay Aging, and the Department of Veterans Affairs. It serves eligible veterans of any age who are at risk for nursing home placement. These services help veterans continue to live in their own homes and communities for as long as possible. The program provides highly qualified and professional staff who will work with you and your family to develop a program of care.

How do I get connected to these services?

To access this service through Senior Service, please reach out to your VA Primary Care Physician to be referred to the Veteran Directed Care Program.
