CDSMP Leader Training

Enhance your health and the health of your community! Get certified as an Evidence-Based Leader for the Stanford University Chronic Disease Self-Management Program (CDSMP). CDSMP workshops teach community members with chronic conditions how to better manage their symptoms and use new techniques for living well. The workshops are held at various sites in South Hampton Roads and Western Tidewater. As a result of completing the training, you will be able to co-lead the 6-week CDSMP workshops in your community.

What to Expect:

  • To qualify for certification completion, you must attend all classes.
  • There is NO FEE.
  • Materials and refreshments will be provided during your training.
